Representative Karen W. Morgan proposes the following amendments:

1.    Page 3, Line 64:     After line 64 insert "(4) An insurer that uses erroneous credit information as a basis for a determination described in Subsection (2)(b) shall be liable for civil damages in an amount equal to $500 for each item of erroneous credit information used as a basis for a determination described in Subsection (2)(b)."
2.     Page 3, Line 65:     Delete "(4)" and insert "(5)"
